Patrón de dislipidemia en los casos nuevos de VIH que iniciaron tratamiento antirretroviral en el primer semestre del año 2017 en el Hospital México de Costa Rica

El VIH es una enfermedad de alta prevalencia en la actualidad. Es una pandemia
global, al final del 2016 había un estimado de 36.7 millones de individuos con la
infección; además en ese mismo año hubo un estimado de 1.8 millones de casos
nuevos de infección por VIH.
El objetivo de este trabajo de investigación es describir las alteraciones que
ocurren en el perfil de lípidos en los casos nuevos de VIH, del Hospital México,
que iniciaron terapia antirretroviral en el primer semestre del 2017 y determinar
la tendencia a través del tiempo a los 6, 12 y 18 meses.
Las alteraciones del metabolismo lipídico varían dependiendo del tipo de TARV.
Existen varias teorías de las causas de este fenómeno como, por ejemplo.
inhibición las proteínas transportadoras de los elementos reguladores de
esteroles (SERBP), la degradación de las apolipoproteínas tipo B y alteraciones
mitocondriales.
Este estudio fue un estudio de tipo observacional descriptivo retrospectivo
mediante revisión de expedientes. Se revisó un total de 125 expedientes de los
cuales se excluyeron 36 pacientes por diversos motivos. De los 89 pacientes que
se incluyeron se documentó que la edad más frecuente fue entre los 30 y 39
años, género masculino en su mayoría, al momento del diagnóstico. La mayoría
presentó un estadio clínico A2, B2 y C3 previo al inicio de tratamiento. El 95.5%
de los pacientes en este estudio tenían un esquema con Efavirenz / Emtricitabina
o Lamivudina /Tenofovir. Respecto al perfil lipídico a los 18 meses, se documentó
hipertrigliceridemia en un 46.4% de los pacientes, HDL menor a 40 mg/dl en un
40.6%, hipercolesterolemia total en 37.7% de los pacientes y un 32.3% de
pacientes con LDL mayor a 130 mg/dl. The human immunodeficiency virus (HIV) is a disease of high prevalence today;

individuals infected. In addition, there was an estimated 1.8 million new cases of
HIV in that same year.
The main objective of this research is describing the variations that occur in the
lipid profile in new HIV cases in Mexico Hospital who initiated antiretroviral
therapy (ART) in the first semester of 2017, describing the trend over time at 6,
12, and 18 months.
Alterations of lipid metabolism vary depending on the type of ART and the drug
combination used. Several theories on the causes of dyslipidemia in these
patients have been proposed; for example, the inhibition of sterile regulatory
elements transport proteins (SERBPs), degradation of type B apolipoproteins,
and mitochondrial abnormalities.
This study was a retrospective descriptive observational study through file review.
A total of 125 files were reviewed and 36 were excluded for various reasons.
For the 89 patients included in this research, the most frequent age at the time of
diagnosis was between 30 and 39 years. Most patients were sorted into clinical
stages A2, B2, and C3 prior to the start of ART. In this study, 95.5% of the patients
were treated with Efavirenz/Emtricitabine or Lamivudine/Tenofovir.
In connection with the lipid profile, hypertriglyceridemia was documented in
46.4% of patients, HDL of less than 40 mg/dl in 40.6% of the patients, total
hypercholesterolemia in 37.7% of patients, and 32.3% of patients presented LDL
levels greater than 130 mg/dl.
