Introduction: Preterm newborns discharged from the Neonatal Intensive Care Unit should be included in a Neonatal Follow-up Program. Continuity of care can be achieved through the Neonatal Pediatric Nursing Home Care. Premature babies have several risks and complications can be prevented during their transition home and readmission to the hospital can be avoided. Additionally, parents of premature babies and their families experience various feelings and challenges with the baby at home. The objective of this article is to analyze the best scientific evidence available on the effectiveness of the Neonatal Pediatric Nursing Home Care for the follow-up of premature infants discharged from Neonatal Intensive Care Units (NICU). Method: Scoping-type review with Evidence-Based Clinical Practice methodology. We searched for studies published from 2014 to 2020, using MeSH terms in: PubMed, CINAHL, Science Direct, MEDLINE and PsycINFO. All types of studies were included and the population was prematurely discharged from the NICU. Articles published in English, Spanish or Portuguese were identified. Results: 13048 research articles were identified. After applying various selection and suitability filters, 9 articles were included for critical analysis evaluation in FLC 3.0, CASPe, and JBI. In the end, 5 studies with the best scientific evidence according to methodological quality were included, and only 2 research articles remained after giving a ranking value to the evidence using the proposal of the Center for Evidence-Based Medicine of Oxford. Conclusions: The evidence shows that Neonatal Pediatric Nursing Home Care programs for the follow-up of premature NICU discharges are effective because: they increase their weight at home without increasing their morbidity; They allow establishing a link between the family and the nursing staff that allows quality care to be provided during the transition from the NICU to the home. Also, it reduces maternal depression and anxiety by improving postnatal care for their babies, minimizing the risk of trauma and aspiration. It even reduces the use of emergency care, especially if the mother has high levels of anxiety. In addition, it is safe because it reduces hospital readmission. Finally, the home follow-up time for premature infants should be greater than six weeks and they require a greater number of nursing home visits as they are at greater risk.es_ES
