Resumen
Objetivo: Analizar el impacto clínico de un programa de monitoreo de farmacoterapia hospitalaria. Métodos: Se analizaron 215 expedientes de pacientes mayores de 18 años con hospitalizaciones de mínimo 24 horas en cuidados intensivos o intermedios, entre enero-mayo de 2018. El impacto del programa se evaluó por la relación entre el número de problemas resueltos y el número total de problemas relacionados con la medicación.Resultados: Se encontró que los pacientes utilizaron un total de 3308 medicamentos. El 69 % de ellos tenían al menos un problema relacionado con la medicación. Se aprobaron el 89 % de las recomendaciones farmacoterapéuticas. Conclusión: El farmacéutico clínico tiene un papel importante dentro del equipo de salud, debido a que pueden mejorar la efectividad, la seguridad y el éxito de la terapia establecida.

Results: A total of 215 patients were included. A total of 3308 medications were used and 69 % of patients had at least one problem related to medication and 89 % of the pharmacotherapeutic recommendations were approved. Conclusion: the clinical pharmacist has an important role within multidisciplinary teams where they can improve the effectiveness, safety and success of the established therapy.
